9:30pm Maureen is sound asleep. She is lying on her left side.
10:00pm Maureen's diaper is medium wet - performed a clean and change. Noted a thick, yellow tinged discharge on the diaper. Repositioned Maureen to her right side.
- Vitals: Temp 97.1, Pulse 58, O2 97%.
12:00am Diaper is dry. Repositioned Maureen to her left side.
2:00am Diaper is dry. Repositioned Maureen to her right side.
4:00am Diaper is medium wet - performed a clean and change. Once again noted a thick, yellow tinged discharge in the diaper. Repositioned Maureen to her left side.
