9:30pm Maureen is sound asleep. She is lying on her left side.
10:00pm Maureen's diaper is dry. No discharge at this time. Turned and repositioned Maureen to her right side.
- Vitals: Temperature 98.6, Pulse 74, O2 97%.
12:00am Diaper is dry. Repositioned Maureen to her left side.
2:00am Diaper is medium wet - performed a clean and change. Thick vaginal discharge. No blood. Repositioned Maureen to her right side.
4:00am Diaper is medium wet - performed a clean and change. Small amount of bright red discharge. Repositioned Maureen to her left side.
