3:45 pm : conversation with the nurse, Maureen was resting in living room,


5:00 pm: transferred Maureen to the kitchen


5:15 pm: dinner: chilli, 3/4 cup of water


6:00 pm: 5mg OLANZAPINE with vanilla pudding


6:15 pm: 7.5ml of VIMPAT


6:20 pm:

Oral care.

Cleaned face, hands, under arms, Inner elbows/forearms, under breast - used Aloe Vesta Cleansing Foam and a warm wash cloth. Applied Nystatin Ointment under right inner forearm/elbow and under right and left breast. Changed t-shirt. Applied Cetaphil to face, arms and hands. Applied Vick’s on throat and chest.


7:00 pm :transferred Maureen to the bed, cleaned & changed in bed (very wet diaper), Cavilon lotion applied, position left side with two wedge pillows. Applied CICLOPIROX to both big toes.


7:30 pm: asleep

8:45 pm: medium wet diaper, cleaned & changed, repositioned right side with two wedge pillows
