  • 8:30am Maureen is sound asleep. She is lying on her left side. Tom says Maureen has been asleep this whole time.

  • 9:00am Maureen coughed once but she is still sound asleep.

  • 9:30am Maureen is sound asleep. We opened the blinds and changed to the Master Mix in hopes Maureen will wake soon.

  • 9:45am Maureen is still sound asleep. Joanna and I repositioned Maureen to her back. She is now slowly stirring, eyes half open.

  • 9:48am Maureen is awake. Joanna and I helped Maureen to a sitting position in bed. Administered 75mcg Levothyroxine with applesauce.

  • 10:00am We begin our day with a partial bed bath using warmed wipes from the purple packet. Maureen's diaper is dry - Joanna and I still performed a clean and change. Applied Cavilon barrier cream to appropriate areas and Cavilon barrier film to the sacral area. During the process, Maureen had a small amount of the same milky white discharge. We dressed and headed to the front room.

  • Vitals: BP 98/58, temp 97.5, pulse 62, O2 96%.

  • 10:33am Breakfast - omelet, banana. 4 oz apple juice/water.

  • Daily MiraLAX dosage - none

  • 11:12am Morning facial care routine.

  • 7 oz lemon water.

  • 11:30am Nebulizer treatment.

  • Maureen is awake, eyes wide open. Lungs clear bilaterally, breathing unlabored. Joanna reported redness on Maureen's sacral area. I assessed the area and staged the redness as a Stage 1 pressure ulcer. Will continue to monitor. Vaginal discharge still present, but no foul odor. Coughing is minimal this morning.

  • 12:00pm We headed to the bedroom for rest break #1. Maureen's diaper is dry. Joanna and I helped Maureen to her right side with two green wedge pillows secured behind her back. Rest break #1 begins at 12:07pm.

  • 12:30pm Maureen is sound asleep.

  • 1:07pm Maureen is still sound asleep.

  • 1:30pm Maureen is still sound asleep but Joanna and I could tell she had a full diaper and decided to get going. Maureen woke up as soon as we repositioned her to her back. Diaper is heavy wet and Maureen had a large, formed bowel movement - Joanna and I performed a clean and change. The pressure ulcer remains at a Stage 1. We dressed and headed to the front room.
