5:30am medium wet, changed, repositioned.

5:40 Given 20mg TROSPIUM

6:30 dry, repositioned.

7:30 very soft healthy BM, light wet, changed, dressed, NYSTATIN on rash under breast.

8:00 Breakfast: banana, prune juice, oatmeal, daily meds plus 5ml OLANZAPINE and 20ml Colace.

Note:. Two swallowing holds after toothbrush.

9:00 PT hands and leg exercises.

9:30 bathroom dry but poop marks. Changed. No tinkling in toilet.

10:00 PT sitting upright exercise.

10:30 Chair to rest.

12noon bathroom medium wet, changed, cloudy tinkle.

12:30 Lunch: ham and cheese on rye, potato salad and mushroom soup and visit from Brian.

2:00 visit from Sister Roberta.

3:20 bathroom, medium wet, changed.
