• 4:00pm Maureen is sitting comfortably watching a game show.

• 4:45pm Checked diaper, it’s dry.

• 4:50pm Dinner: Spaghettis with sausage, and half cup of water. Coughed once.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a medium wet diaper, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities. Clean/dry redness spot by her right armpit, applied nystatin powder.

• 6:50pm Positioned on her left, Maureen goes to sleep for the night.

• Vitals: temp: 97.3, pulse: 72, O2: 97.
