• 4:00pm Maureen sits comfortably in the recliner watching games show.

• 4:40pm Blood pressure: 110/75, P 75.

• 4:45pm Checked diaper, it’s dry.

• 5:00pm Dinner: Spaghetti with chicken, prepared by Tom, and half cup of lemon water. Maureen enjoyed very much! Coughed once.

• 5:50pm Administered evening meds: 12.5ml Vimpat, 5mg Loranzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Cleaned and changed a medium wet diaper, applied Cavilon barrier cream to sacral and inner thighs area. Wiped clean hands and face, applied Cetaphil cream, including lower extremities.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.

• Vitals: Temp: 97.1, Pulse: 71, O2: 98.
