• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 108/60, P64.

• 4:45pm Diaper is dry.

• 4:50pm Dinner: Rigatoni with sausage, mushrooms, peas and tomato, and 6oz of lemon water. Coughed twice.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6;00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper with a medium-soft BM and some vaginal discharge, applied Cavilon barrier cream , applied nystatin powder to left inner thigh fading redness, abdomen area is clear. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.
