4:00 PM. Arrived. Maureen is sitting comfortably on her recliner chair watching Game show.

**giving her sips of water in between. Coughed 2x

5:00 PM. Dinner- chicken pesto and Italian noodles with peppers with lemon water which Maureen enjoyed. **coughed 1x

5:45 PM. BP 125/63, pulse is 53 bpm

6:00 PM. Administered 5mg of Olanzapine and 12.5 ml of Vimpat with apple sauce.

6:15 PM. Oral and denture care done.

6:30 PM. Transferred Maureen back to bedroom. Diaper was heavy wet. Cleaned, dried and changed. Applied Cavilon cream to perineal area and Cetaphil lotion to upper and lower extremities. Positioned her to be facing left side with 2 wedge pillows and put a pillow in between her legs to avoid any pressure injury.
