• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 103/61, P76.

• 4:50pm Diaper is light wet.

• 5:00pm Dinner: Beef casserole and half cup of lemon water. Coughed three times, suction pump used.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a medium wet diaper with small vaginal discharge, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities. Left arm pleat redness is fading, clean/dry, applied nystatin powder.

  Pulse: 83

  O2: 97

• Positioned on her left side, Maureen goes to sleep for the night.

• Maureen comsumed 38 ounces of fluid.
