• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 110/69.

• 4:45pm Diaper it’s light wet.

• 4:50pm Dinner: Cod with tomato and olives, couscous with sweet peas, and 1/3 cup of lemon water. Coughed three times.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper with Small-formed BM and small-clear vaginal discharge, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.

• Vitals: temp: 96.8, pulse: 70, O2: 97.
