• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 110/64, P67.

  O2: 97.

• 4:45pm Salina Neb treatment.

• 4:50pm Diaper is dry.

• 5:00pm Dinner: pasta with cheese, peas and tomato, and 6oz of lemon water. Maureen coughed four times during meal, suction pump used once.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

  6:00pm Oral care.

  6:20pm Transferred Maureen to the bedroom. Barb and I clean/change a medium wet diaper, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

  Positioned on her left side, Maureen goes to sleep for the night.
