• 4:00pm Maureen’s sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 108/60, P 74.

• 4:45pm Diapers is dry.

• 4:50pm Dinner: Chicken with sundried tomato sauce, cauliflower and beans, and half cup of water. Coughed once.

• 5:45pm Administered 800mg Acyclovir (crushed) with apple sauce.

• 6:00pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:10pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a medium wet diaper with a Large-loosed BM (a little runny), applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.
