Monday April 22,2024 Swing shift

• 4:00pm Maureen is sitting comfortably in her recliner watching games show.

• 4:30pm Checked diaper, it’s soiled. We transferred to the bedroom to change. Diaper was medium wet with a Medium-soft BM, cleaned/changed, then headed back to the living room.

• 5:00pm Dinner: Sloppy Joe’s with mashed sweet potatoes, cauliflower, and half cup of lemon water.

• 5:50pm Administered 12.5ml Vimpat, 5mg Olanzapine with apple sauce.

• 6:00pm Evening oral care.

• 6:20pm Transferred Maureen to the bedroom. Diaper was medium wet, cleaned/changed, applied Cavilon barrier cream to sacrum and inner thighs area. Wiped clean hands and face, applied Cetaphil cream, including lower extremities. Cleaned, dried and applied nystatin powder to redness by her left armpit.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.

• Vitals: Temp: 97.1, Pulse: 74, O2: 97.
