Ativan dosage for Sunday was .5mg at 5am & .25 at 1:20PM



Bed, 7:30

Asleep, 8:00

Cleaned and changed at 11, 2, and 5:30, wet each time.

Zinc oxide applied to spot at base of tailbone


5:45AM, .5mg Ativan given
