Friday, May 30, 2025 Swing shift

  • 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

  • 4:30pm Blood pressure: 109/70, P76.

  • 4:45pm Diaper is dry.

  • 4:50pm Dinner: Spaghetti with meatballs, and 4oz of lemon water. She coughed twice and sneezed once during meal.

  • 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

  • 6:00pm Oral care.

  • 6:20pm Transferred Maureen to the bedroom. Tom and I clean/change a heavy wet diaper with a small brownish vaginal discharge, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

  • Vitals:

    Temp: 97.3

    Pulse: 73

    O2: 97

  • Positioned on her left side, Maureen goes to sleep for the night.

Wednesday March 4th Daytime Summary

-9:30am Maureen is awake. Tom gave her a couple of swabs of water. We put her in a upright position & she drank 6oz. of water. We gave her 75mcg of Levothyroxine w/applesauce. Also albuterol breathing

10:00pm, very wet, cleaned, changed, reposition right. Urine smells like poop 12:00am, Dry, reposition left 3:00am, dry, reposition right 5:00am, medium wet, cleaned, changed 5:40am, dry, reposition
