• 4:00pm Maureen’s sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 110:64, P 71.’

• 4:45pm Diaper is dry.

• 5:10pm Dinner: Pizza with half cup of water. Coughed twice.

• 6:00pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:10pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Diaper is dry and clear. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.
