CLM-I checked with Tom and Maureen was on Oxcarbazepine, the liquid, from 1/20 to 1/26. She had lost her motor skills, could not stand.
Is there another approach?
CLM-My previous note recapped our concern with loss of motor skills when taking Oxcarbazepine. I seem to recall that you may have mentioned that it could be a result of the UTI's. Attached is the recent history of UTI's. We have a call with dr. Purinson on UTI's today and Dr. Sand on Tuesday. The blood draw is now scheduled for Monday.We will await your direction on meds.
Dr. Yucus-Patients with dementia commonly have loss of function, such as loss of motor skills, when they have a urinary tract infection. I don't think we gave the oxcarbazepine a fair trial, and since she is getting more sleepy after each dose of the Depakote sprinkles, I want to try the oxcarbazepine again to give it a fair trial to determine if it will help to prevent seizures. If there is a loss of motor skills without a urinary tract infection, then I will change the oxcarbazepine to a different medication.
CLM-Further perspective. She took three hours after the seizure to wake. Had some food, then fell asleep. We could not wake her and she slept thru the night. She did not get nighttime sprinkles. After breakfast today, she had sprinkles. She is not sleepy. She is joyful and engaging. So, should we change the medicine?
Dr. Yucus- It is possible that her blood level of Depakote is lower, since she did not receive the dose of medication last night, and this is why she is not sleepy after this morning's dose, since there is less medication in her body. In regards to your other message from today, below are my instructions for switching medications:
If she still has the oxcarbazepine liquid, she may take the following:
2.5 mL (150mg) daily for 1 week, then
2.5 mL (150mg) twice daily for 1 week, then
2.5 mL (150mg) every morning and 5mL (300mg) every evening for 1 week, then
5 mL (300mg) twice daily
At the same time, she may decrease the Depakote sprinkles to 2 capsules daily for 1 week and then stop.
